Complete export format

Why these files

ONC electronic health information export asks for an electronic, computable copy plus a public format page. Clinical chart files are PDF. Rosters and billing ledgers are UTF-8 CSV. Uploaded originals keep their original type. Word is not the chart format.

Folder names match the TrustRoom product. They are not “Medical Records” or “Psychotherapy Notes.” Notes in this pack are chart notes in the designated record set, not psychotherapy notes under 45 C.F.R. § 164.501.

CSV columns

Encoding is UTF-8. Fields with commas are quoted. Amounts are dollars with two decimals.

client_name, date, amount, payment_status, billing_type, invoice_number, payment_id

What is not in the zip

Calendar appointments are not included. Plan templates and weekly schedules are omitted; only scores and written answers are kept. Session audio is not retained.
